PMHCC Case Management provides case management services to individuals with dual diagnoses of substance use and mental health disorder with the hope of sustaining a self-fulfilling recovery lifestyle. Our services focus on promoting autonomy and self-advocacy and are based on case management principles that are client-centered, community-based, pragmatic, flexible and culturally sensitive. Case managers align these principals to provide supportive ancillary services that drive engagement, service coordination, goalsetting, monitoring and coaching.

Case managers support all stages of recovery. Case managers take the time to understand individual’s needs, evaluate strengths and challenges, and create a plan that will help individuals access the support and community resources they need to achieve their self-defined goals. Our case managers are as creative and innovative as necessary to tailor the recovery needs of each individual to achieve long-term recovery.

The Assertive Outreach Team

The Assertive Outreach Team, also known as The AOT Team, is one of our specialty teams specializing in enhanced cases with opioid use disorder, and chronic homelessness with the focus of creating stability and navigating recover with the incorporation of their Primary DOC being Opioids and addressing the on-going concerns with Homelessness by connecting them to vital resources.

The FIT Team

The Families in Transition Teams also known as the FIT Team specializes in pregnant woman, women/men with children’s facilities, DHS involvement with the focus of creating stability and navigation recovery with the incorporation of how to navigate the family aspect of their individual lives.

CORE

The Community Outreach and Engagement Team also know as The CORE Team specializes in the older adults and the high medical needs population with the focus of creating stability and navigating recovery with the incorporation of their higher level of needs for medical support and important resources for the aging population.
