Who We Serve
‍PMHCC provides case management services to eligible Philadelphia residents experiencing substance use disorders who require coordination across treatment and community support.

‍We accept referrals from:

1. Submit a Referral: Complete the secure online referral form below or contact our Intake Coordinator directly.

2. Referral Review: Referrals are reviewed within 5 business days to determine preliminary eligibility.

3. Initial Contact: The Intake/Discharge Specialist will contact the referral source (and/or the individual, as appropriate) to:
