PMHCC Case Management

PMHCC Case Management provides case management services to recipients of BHSI funding (uninsured or underinsured) and women with children enrolled in Community Behavioral Health, all of whom have substance abuse disorders or may have a co-occurring disorder (mental illness and addiction). Case managers support clients and promote a drug-free and sober life-style by connecting them to a range of community resources, mentoring and advocacy.

Based on need, persons with substance abuse disorders and eligible for CBH (HealthChoices) insurance may be referred for PMHCC Case Management services by all DBH Providers. Persons seeking self sufficiency and recovery from substance abuse and co-occurring disorders are provided ongoing support and ancillary services such as: engagement, evaluation of the client’s strengths and needs, service planning and goal setting, linking, monitoring, advocacy and coaching.
