CTT COVID-19 Employee screener

As CTT expands on-site operations, all of us share a responsibility to the City and our fellow employees to ensure that our return to on-site work is done in a safe manner and does not create opportunities for further spread of the COVID-19 virus. To help with those efforts, Staff are required to complete the following COVID-19 symptom assessment tool which is intended to determine if it is safe for an employee to work on-site or in the community.

Here is how to complete the assessment tool:

  1. Complete the assessment prior to your arrival onsite for work.
  2. Respond to each question truthfully and to the best of your ability.
  3. Follow the prompts provided at the end of the assessment - this may include communicating with  your supervisor and Primary Care Provider about any follow-up actions you need to take after receiving your results.


Confidentiality disclaimer:

CTT places a high priority on maintaining all employee health information in a confidential manner. Accordingly, CTT will not store employee responses to this self-administered assessment tool. Information relating to an employee's absence from work, including, if necessary, the reason for that absence and the expected duration of any absence, will continue to be collected and maintained according to existing departmental protocols.

Have you experienced any of the following symptoms in the past 48 hours:

Within the past 5 days, have you been in close physical contact (6 feet or closer for at least 15 minutes within a 24 hour period) with a person who is known to have laboratory-confirmed COVID-19 or with anyone who has any symptoms consistent with COVID-19?

Has a health care professional recommended you isolate or quarantine because you may have been exposed to a person with COVID-19 or are worried that you may be sick with COVID-19?
